Entry Form 2006
• The Entry Form requires title, author, ISBN, and publication date (month and year).
• Make sure each title complies with the certification called for in Section 1.4.
• Please do not abbreviate. The exact format in which the title and author is submitted will be the format used in all publicity regarding Gold Medallion finalists/winners.
• You will also be asked to upload to our FTP site, the company logo of the book publisher and a 300 dpi high resolution jpg digital file of the book cover.

Payment
After you submit your entry form online, you will receive an automatic confirmation which will contain your payment form. Please print out the payment form, fill it out and mail it with your check to ECPA. Your signed payment form certifies that your entries are in compliance with the 2006 ECPA Christian Book Awards official rules. Entry forms and payment must be received no later than January 17, 2006.
Shipping Books
After you have submitted your entry and payment forms by January 17, 2006 please prepare to ship copies of the entered titles February 1-13 to: Six (6) copies each entered title are to be shipped to: Ingram Book Group Attn: Judy Allen / ECPA Christian Book Awards 14 Ingram Blvd., La Vergne, TN 37086 One (1) copy of each entered title is to be shipped to: ECPA / ECPA Christian Book Awards 9633 S. 48th Street, Suite 140, Phoenix, AZ 85044
